Quality Improvement Advisor – Diabetes Educator (Montgomery, AL Area) – Quality improvement team needs individual to coordinate and implement Diabetes Self-Management Education (DSME) curriculum with Medicare beneficiaries in Alabama. Other clinical and quality improvement activities will be required with designated providers, stakeholders and partners to help achieve better health care, better health, and reduce costs of health care through continual improvement. Activities may include evaluating appropriateness, efficiency and effectiveness of the DSME program, and recommending improvements through on-site visits, meetings, phone consultation and abstraction of data in collaboration with the team. Must be able to function independently yet work effectively in team environment. Bachelor’s degree in health related profession required; Master’s degree preferred. 2+ years of past experience in health education field preferred. Advanced levels skills in Microsoft Suite, strong communication and organization skills required. Travel statewide to provider/practitioner sites as needed. Excellent benefits. Survey Specialist (2 part-time positions: 1st – 10:00am-2:00pm M-F, 2nd – 3:00-7:00pm M-F) –
This position is part of a tobacco cessation program dedicated to providing counseling, education on tobacco, health information and product knowledge. The Specialist will be responsible for contacting those cases on the survey tickler file, entering basic information in the database according to the survey questions, and scheduling call backs for the next survey contact call. Specific duties and responsibilities include documentation of each call and completing surveys on designated calls. Specialist must be able to commit to the hours needed to complete the assignment. Applicant must be detail oriented, able to work independently, able to focus on details for extended periods of time and adapt to changing timelines, priorities, and guidelines. Specialist must have good verbal and listening skills and basic writing and word processing skills. Knowledge of cessation treatment protocol, treatment delivery options and familiarity with confidentiality policies preferred. Previous customer service experience in a call center environment preferred. High school diploma or equivalent required; some college coursework preferred. Specialist may not presently be a smoker or tobacco user, may be in cessation treatment or have been through tobacco cessation treatment, may be a former smoker or tobacco user, may be someone who has never used tobacco.
