Community Health Needs Assessment
Every community is unique. And every community has unique health needs. We examine the environment, history, local habits, socio-economic statistics – anything and everything that affects the collective health of the people in your area. Then, we give you a detailed report that will tell you what to be prepared for now and in the future. Here’s a look at our step-by-step process.
Step 1: Establish the Infrastructure.
To begin, we make sure all the players are at the table. This is typically the medical centers along with any community partners that may be offering support.
Step 2: Determine the Purpose and Scope.
This is where we can define more specifically what we’re looking for out of your assessment.
Step 3: Collect and Interpret Data.
Censuses, surveys, focus groups. During this step we go over all the information we can find that applies to the population in your area.
Step 4: Define and Validate Priorities.
This is when we note which health needs most impact the community as a whole.
Step 5: Document and Communicate Results.
When everything’s been noted, we put it all in a detailed report for stakeholders and interested parties to see.
Step 6: Develop an Implementation Strategy.
After identifying what issues confront your community, we will offer solutions to meet your specific needs.
Community Benefit Plan
Once we complete step six, you will receive a results-oriented Community Benefit Plan to improve the overall health of the people in your area. Along with local stakeholders, you’ll have a blueprint for addressing unmet needs, promoting healthy behaviors, reducing the burden of illness, and more.
